Adult ASD (Autism Spectrum Disorder) referrals

How to start a referral request:

1. Patients who wish to be referred must first visit each of the provider websites (Right to Choose options below) to ensure that they have read and finalise their decision. (Patients are only able to be referred to one service at the time of referral.)
2. Complete the relevant patient questionnaire for your chosen provider and send it to us to be saved on your patient notes. bnssg.bridgeviewmedical@nhs.net 
3. It is now time to book an appointment with your GP to discuss being referred for an ASD assessment. If your GP feels that a referral is appropriate, they will organise for a referral form signed by them, to be sent to the relevant service provider along with any attachments as necessary.
