Accessing your medical details
As a patient you have a right to view your health record, such as the information we hold about you and your medical history, including any conditions or allergies you have and any medicine you’re taking. You can also view letters, test results and appointment notes.
How to get your medical details
You can access your medical details with your NHS number on the How to get your medical records page on the NHS website.
We also recommend that you use the NHS app to access your medical health record. To download the app or for more information, go to NHS App.

If you have a specific request for information please use the KLINIK system to request administrative attention.
Please note that if you are requesting copies of medical notes you are required to complete a consent form and present it with proof of identity to reception before we can act on your request.
Accepted forms of ID include: Photo ID, passport, driving licence, original birth certificate; in addition to a utility bill or official correspondence with your name and home address on.
Requests received by post
When a postal request is received from yourself or an outside agency requesting medical information we will contact you and ask you to complete a consent form to enable us to act on the request.
All requests will be processed accordingly and you will be notified when you can collect your copies, unless prior arrangements have been made.
Requests for copies of notes can take up to one calendar month to complete.
Some services available are not covered under our contract with the NHS. This means that these services need to be paid for.
The services that include charges are:
- Medicals for pre-employment, sports and driving requirements
- Insurance claim forms
- Prescriptions for taking medication abroad
- Private sick notes
- Letters written by a GP

Patient Confidentiality
Health and Care records are confidential. If you are requesting information about a patient please be aware we will only release information if we have patient consent such as written consent from the patient, or evidence of a legal authority such as power of attorney.
