Social Prescribing - Diet and lifestyle
Social prescribing is an NHS model that helps connect you with the community to manage your health and wellbeing. There are lots of people, activities, groups and services in the community that can help meet your needs. For example, volunteering roles, cooking classes, sports and more.
If you need support to link into any of the services or activities listed below, please ask your GP for a referral to our social prescribing service. You can also self-refer here https://bridgeviewmedical.nhs.uk/patient-care-support/social-prescribing or by emailing leanne.purton@nhs.net
We offer up to 6 appointments, exploring what matters most to you and helping you to connect with services, activities and support available locally.
