Care Co-ordinators
We are open to receive applications for full time or part time work if you can meet either of the following working patterns:
Part time: Monday - Friday PM: 1:15pm to 6:45pm (minimum 3 afternoons per week).
Full time: 40 or 37.5 hours per week Monday - Friday
8:15am to 6:45pm (4 days x 10 hour working day with a 30-minute unpaid lunch break) OR
10:45am to 6:45pm Monday - Friday with a 30-minute unpaid lunch break.

Job summary
Care Coordinators (traditionally GP Receptionists) assist directing patients to our appropriate services and healthcare professionals.
You will be polite and positive, enjoy supporting patients, healthcare partners and your colleagues in person or via the telephone.
You will also complete some administrative work to ensure our services run effectively, so an eye for detail and being computer literate is essential.
The details
- Full time or part time positions available to cover the hours 8:15am-6:45pm. Part time shift patterns are: AM 8.15am-1.15pm PM: 1.15pm-6.45pm
- You will be expected to work across all of our four sites when needed. All our sites are within 2 miles of each other
- The role starts at £11 per hour, rising to £11.55/hr after 12 months.
- You will receive 35 days annual leave including Bank Holidays (pro-rated for part-time hours, so 20 hours a week role generates 15 days annually +Bank Holidays)
- You can join the NHS pension scheme.
- You will receive comprehensive 1 to1 training, along with ongoing support.
Job responsibilities
- You are the first point of contact for patients via the phone or using our online booking system; navigating them to the appropriate care/ treatment, using the guidelines provided
- You support patients arriving at our practices, dealing with their wide range of enquires and ensuring they receive the appropriate care
- You manage communications to GPs via existing systems
- You ensure appropriate documents are ready for each GP surgeries
- You manage written, verbal and electronic communication in a diplomatic, empathetic, supportive and informative way, especially when dealing with sensitive information
- You complete digital administrational tasks to support the delivery of the clinical service.
Skills required
- Very good communicator – ability to adapt to different situations; being sensitive when dealing with discreet situations
- Good emotional intelligence to calmly manage distressed patients and their demands
- Strong resilience
- Ability to continually learn
- Attention to detail to follow guidelines and complete tasks
- Strong IT skills
Person specification
Essential
- GCSE with English at Level C and above
Desirable
- Customer care, reception and administrative experience
- Experience of working in health care services
