Click on one of the links below and complete a simple online registration form. You do not need proof of address, immigration status, ID or NHS number. You can choose a site to register with, but Bridge View Medical operates as one practice so you may be asked to attend one of the other sites. They are all within one mile.

You do not need to show ID, proof of address or immigration status to register as a patient. 

Please check your answers carefully and ensure you have completed all the questions. Missing information or incorrect information slows down the registration process as our staff will need to contact you to gain the correct details. 

Our catchment area

You can only Register with us if you live within the Bridge View Medical Practice Boundary. To find out whether your address falls within this area please see the map via this link or alternatively the registration team can be contacted via email bnssg.bvmregistrations@nhs.net. 

 

Registering online as a non-English speaker

You can find guidance and advice on how to become a new patient if you’re first language is not English, or if you’re an asylum seeker. You can find out more information about the NHS and how to register and access emergency services on the NHS website.

Registering as a temporary patient

If you are ill while away from home or if you are not registered with a doctor but need to see one, you can receive emergency treatment from a local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

To do this, please complete the following form from and hand into one of our Surgeries. You can collect hard copies of this from us too.

Change of address

If you are moving house, please let us know as soon as possible. If you are moving out of the surgery’s catchment area, you may be asked to register with a surgery that’s closer to your new home address.

You can contact us to let us know about a change in your address.

Please also let us know if your telephone number or email address changes.
